The nut that comes with the set is to big for the screw, I could see light between the threads of the two parts. This is why so many people have had the bolt threads strip and the tool fail.
I purchased a 3/4 - 10 coupler nut for ~$8 at a hardware store, now the nut and bolt thread together correctly and the tool works properly.
